Pamela Cantor MD and URL—www.pamelacantormd.com (the “Website”) (collectively, “Dr. Cantor,” “we,” “us” or “our”) recognizes the importance of safeguarding the personal information provided to us by the users of our Website (“Users,” “you” or “your”). Dr. Cantor is committed to safeguarding that personal information and has implemented this privacy policy (the “Policy”) and internal policies and practices to keep your personal information secure.

The Policy covers our treatment and use of personally identifiable information and/or personal data that is collected via the Website (the “Personal Information”) from Users when such Users electronically communicate with or otherwise access the Website. This Privacy Policy covers only Personal Information that is collected through the Website and does not govern Personal Information collected from any other source or in any other manner.

Collection and Use of Information

The Personal Information We Collect. This Policy applies to all information collected by or submitted to Dr. Cantor via the Website. We consider “personal information” to mean information that identifies, relates to, describes, references, is capable of being associated with or could reasonably be linked, directly or indirectly, with a particular consumer or device. Personal information does not generally include de-identified or aggregate information, or public information lawfully available from government records. For example, when you sign up to receive articles or information about upcoming events or submit questions to Dr. Cantor through the Website, you will be asked to provide a variety of information including, but not necessarily limited to, your name, email address, and phone number. Dr. Cantor retains this information unless and until you request deletion of this information or as otherwise provided in this Policy.

Financial Information. We do not collect financial information from users.

IP Address. An IP (Internet Protocol) address is a number that is automatically assigned to your computer by your internet service provider when you log on. Your IP address is not linked to your Personal Information but Dr. Cantor does reserve the right to use IP addresses to identify individuals who threaten the Website or other users or visitors. IP addresses may also be used to help diagnose problems with the Website and to gather broad demographic information.

Other Information. When you access the Website, Dr. Cantor will automatically receive some information about your use of the service. The type of information that is automatically collected by Dr. Cantor includes such things as the date and time of access of the Website, the Internet protocol (IP) address of the computer that you are using, the domain and host from which you access the Internet, your browser software and operating system, the referring website address, and the kinds of activities you are engaged in while using the Website. This information is used to help us improve and enhance the Website to better meet the needs of its users.

Dr. Cantor contracts with non-affiliated third parties/business partners to host the Dr. Cantor and to provide data collection and reports regarding users’ activities on the Website. These non-affiliated third parties/business partners use cookies on our behalf and receive information about the Users browsing activity and collects information anonymously (meaning without attaching it to any personally identifiable user) and aggregates information collected from Users. These non-affiliated third parties/business partners have agreed not to use this information other than (i) in connection with providing services to us to make our Website more useful and efficient, (ii) as permitted in this Privacy Policy, or (iii) as otherwise permitted by law.

Dr. Cantor will also aggregate Personal Information and may use, share and disclose such aggregated Personal Information as permitted by law. For example, we may use such aggregated Personal Information for research and product development purposes, and may also share such aggregated Personal Information with Dr. Cantor’s business partners.

Regardless of whether a User has chosen to limit Dr. Cantor’s sharing of Personal Information as collected on the Website, Dr. Cantor may, without restriction, use, share and disclose aggregated statistics about Users, along with specific information concerning individual Users that is not Personal Information in any way permitted by applicable law.

Dr. Cantor uses an analytics tool called Google Analytics that helps website and mobile application owners understand how visitors engage with their websites and devices. We can view a variety of reports about how visitors interact with our Website so we can improve it. Google Analytics collects information anonymously. Google Analytics uses cookies, and IP addresses, and reports website trends without identifying individual visitors. In the event a visitor to the Website voluntarily submits contact information to Google Analytics, that company may then associate such information with the applicable IP address to enable us to see that visitor’s specific interactions with our Website. Do Not Track Signals

Retention of Personal Information. Personal Information is retained to deliver our services, to otherwise fulfill identified or apparent purposes, and Personal Information may be retained for record-keeping. At some point Dr. Cantor may elect to destroy, erase or anonymize your Personal Information. Once anonymized, we undertake not to re-identify the information. Our Website also includes social media features, such as the Facebook “like” button and widgets, the “Share this” button or other interactive mini-programs that run on our site. Social media features and widgets are either hosted by a third party or hosted directly on our Website. Information Storage and Protection

Dr. Cantor will maintain reasonable procedures to protect the security and confidentiality of Personal Information provided to it by Users, in accordance with applicable Law.

There are industry standard safeguards in place to protect Users’ Personal Information, such as the use of firewalls and SSL certificates, and limiting access to such information by company personnel. Personal Information will be stored in databases and servers located in the United States. However, no transmission made through the internet can be made completely secure, and accordingly, we will not be liable for the disclosure of any Users’ Personal Information that occurs as a result of transmission errors or the actions of any third party to the fullest extent permitted by law.

In the unlikely event that we determine that there may have been a security breach that results in the unauthorized disclosure of Personal Information to a third party, Dr. Cantor will promptly notify consumers whose Personal Information it reasonably believes may have been compromised by posting information on the Website and, if email contact is possible, then also by sending an email notification with respect to the situation.


Options to Review and Opt-Out Choices

Your Rights and Choices

Right To Know—You have the right to request that we disclose certain information to you about our collection and use of your Personal Information over the past 12 months. Right To Delete Personal Information—You have the right to request that we delete any of your personal information that we collected from you and retained, subject to certain exceptions. Once we receive and confirm your request to delete your personal information, we will delete (and direct our service providers and contractors to delete) such information from our records, unless an exception applies.

We may deny your deletion request if retaining the information is necessary for us or our service provider(s) or contractors to:

Once we receive and confirm your request to correct your Personal Information, we will correct (and direct our service providers and contractors to correct) such information from our records, unless an exception applies.

Verifiable Consumer Requests—If you submit a request to us to disclose how we collect and use your Personal Information or to delete your Personal Information from our records, we must be able to verify your request. There is no charge to you for us respond to your verifiable consumer request, unless it is excessive, repetitive, or manifestly unfounded. If we determine that the request warrants a fee, we will tell you why we made that decision and provide you with a cost estimate before completing.

We will only use Personal Information provided in a verifiable consumer request to verify the requestor’s identity or authority to make the request.

Right to Non-Discrimination—We will not discriminate against you for exercising any of these rights. Accordingly, by way of example, we will not deny you goods or services; charge you different prices or rates for goods or services, including through granting discounts or other benefits, or impose penalties; give you a different level or quality of goods or services; or suggest that you may receive a different price or rate for goods or services or a different level or quality of goods or services.

We may, however, offer you certain financial incentives permitted by applicable law that can result in different prices, rates, or quality levels provided that such incentive is reasonably related to the value of the Personal Information.
